What do I need to bring to my primary care appointment?

  • Photo ID
  • Current insurance card(s)
  • Co-payment(s), if required
  • List of current medications
  • If you have Medicare, please bring your pharmacy insurance drug card.

What is your primary care appointment cancellation policy?

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask you to give us 24 hours advance notice in order to provide an opportunity for another patient to be seen.

What are your billing requirements?

Patients must pay any co-payments at the time of their visit. Patients are responsible to pay any amounts, such as deductibles, co-payments and co-insurance that their insurance company designates is their responsibility.

What should I do if I have a question about my bill?

What do I do if my prescription runs out before my next office visit?

Please call the office before your prescription runs out. We may be able to refill before your next visit, but we ask that you allow 24-48 hours for refills.

What should I do if your office is closed and I need medical care?

If you are experiencing what you believe to be is a medical emergency, please call 9-1-1 or go to the closest hospital Emergency Department. To page the on-call physician after hour for urgent matters, please call the answering service at the main office phone number (740) 615-1800.
